Prison Break season 1 follows two brothers caught in an extraordinary situation. Lincoln Burrows (Dominic Purcell) stands convicted and sentenced to death for a crime he steadfastly claims he never committed. His younger brother Michael Scofield (Wentworth Miller) possesses a brilliant mind and takes an extreme step—deliberately getting himself imprisoned to carry out an elaborate escape plan. Michael orchestrates his own incarceration at Fox River State Penitentiary, the same facility where Lincoln awaits execution for the alleged murder of Terrence Steadman (John Billingsley), the Vice President’s brother.

Inside Fox River, Michael quickly becomes entangled with several pivotal characters. His arrival creates immediate tension with Correctional Officer Brad Bellick (Wade Williams). He shares a cell with Fernando Sucre (Amaury Nolasco) and strategically approaches John Abruzzi (John Abruzzi), manipulating his way into Prison Industries by dangling information about Fibonacci, a witness whose testimony threatens Abruzzi’s freedom.

Michael also forms connections with Charles Westmoreland (Muse Watson), who many believe to be the legendary D.B. Cooper. His plan involves regular visits to Dr. Sara Tancredi (Sarah Wayne Callies) under the guise of needing insulin injections for Type 1 diabetes. Additionally, he strikes a deal with Benjamin Miles “C-Note” Franklin (Rockmond Dunbar) to secure necessary medications for his scheme.

While working on a Taj Mahal replica for Warden Pope (Stacy Keach), Scofield reunites with Lincoln and reveals that his elaborate body tattoo contains the prison’s blueprint. Meanwhile, Lincoln’s son LJ (Marshall Allman) faces imprisonment on drug charges, and evidence emerges linking a conspiracy against Lincoln to higher powers. Veronica Donovan (Robin Tunney), Lincoln’s former girlfriend and attorney, begins investigating his case, convinced of his innocence.

The plot thickens as various challenges emerge. Sucre is sent to solitary confinement for contraband possession, while Scofield needs to acquire an allen bolt from Theodore “T-Bag” Bagwell (Robert Knepper), who uses intimidation tactics rather than sexual favors in exchange for assistance. Dr. Tancredi grows suspicious of Scofield’s diabetes and orders testing. During a racial conflict between inmates, Scofield manages to obtain the bolt and remove his cell toilet. T-Bag suspects Scofield of murdering his lover, while Franklin helps Scofield maintain his medical ruse, ensuring continued access to the infirmary.

The narrative grows more complex as Abruzzi’s associate pushes him to locate Fibonacci, and though Bellick inspects Scofield’s cell, he finds nothing suspicious. Lincoln reveals to Donovan that the incriminating tape was fabricated, leading her to track down a witness named Leticia Barris in Chicago. Secret Service agent Paul Kellerman (Paul Adelstein) monitors Donovan’s investigation but is not directly involved at this stage. Meanwhile, Sucre’s relationship with his fiancée Maricruz Delgado (Camille Guaty) becomes strained when she begins seeing his cousin Hector (Kurt Caceres) during his confinement.

Tensions escalate as Abruzzi’s men torture Scofield for Fibonacci’s location. Lincoln, burning with rage over Abruzzi’s previous attack on his younger brother, yearns for immediate revenge. However, Lincoln’s threats are tempered by Scofield’s reminder that they need Abruzzi’s cooperation. Upon Sucre’s release from solitary, Scofield tests his loyalty before revealing his plans. Though Sucre initially passes the test, he considers requesting a cell transfer to avoid complications before his upcoming release. Abruzzi realizes he must befriend Scofield to locate Fibonacci and intervenes when T-Bag attempts to lynch Scofield, resulting in T-Bag’s isolation.

The plot continues to unfold as Scofield shares his escape plan with Abruzzi while LJ reunites with Lincoln through his probation officer’s intervention. Kellerman and his partner Agent Hale (Danny McCarthy) eliminate Barris before she can provide testimony, prompting Donovan to leave her fiancé Sebastian Balfour (Anthony Starke). New complications arise when Bellick assigns Charles “Haywire” Patoshik (Silas Weir Mitchell) to Scofield’s cell. Haywire becomes fixated on Scofield’s tattoo, believing it represents a path, forcing Scofield and Sucre to devise a plan for his removal.

When Sucre discovers Hector’s advances toward Maricruz, he commits to Scofield’s escape plan but must first help eliminate Haywire. Using chemicals from Abruzzi and Haywire’s toothpaste, Scofield creates an opening in the infirmary. The guards remove Haywire, allowing Sucre to return as Scofield’s cellmate while they tunnel beneath the cell toilet into hidden passages of the prison.

As the escape plan progresses, Donovan approaches Project Justice for help but gains only Nick Savrinn’s (Frank Grillo) support. Kellerman and Hale discover the brothers’ relationship and attempt to transfer Scofield. However, Pope resists until threatened with exposure regarding his illegitimate son. Westmoreland advises Scofield to file a court petition to delay the transfer, though operatives pressure Pope to dismiss it. Scofield cleverly determines their escape route by analyzing police presence around the prison’s surrounding streets.

The situation becomes increasingly dire when T-Bag is released from solitary confinement and discovers the escape plot, threatening exposure unless included. Scofield attempts to buy more tunneling time by orchestrating a lockdown. However, T-Bag escalates it into a full riot. During the chaos, Dr. Tancredi’s life is endangered by Maximum Security inmates, forcing Scofield to jeopardize his escape plan to save her from assault while Kellerman and Hale recruit an inmate named Turk (John Turk) to assassinate Lincoln, leading him into an abandoned underground area.

Outside the prison, Donovan initially maintains distance from Savrinn, but his credibility grows when he uncovers that the anonymous call leading to Lincoln’s arrest originated from Washington, D.C., despite the alleged murder taking place in Chicago. As tension mounts at the prison, Vice President Caroline Reynolds (Patricia Wettig) arrives with intentions to storm it, gaining Bellick’s support but facing opposition from Warden Pope. During this crisis, Scofield rescues Sara from dangerous inmates in the infirmary, and after an intense struggle, she escapes to safety while he returns to his cell. Sara grows suspicious, realizing Scofield’s presence in the infirmary seems implausible.

Meanwhile, Sucre and Abruzzi continue their tunneling efforts, nearly triggering a gas explosion before finally breaking through the wall. Lincoln manages to kill Turk during an assassination attempt but fails to uncover who ordered the hit. Despite warnings, T-Bag murders a corrections officer without any leverage involving photographs. In Washington, D.C., Donovan and Savrinn trace the anonymous call to a phone booth outside an abandoned building, only to receive an ominous threat to their lives.

The escape plan requires access to the guards’ break room, but entry is restricted to select inmates, including Westmoreland, who initially refuses to participate. When COs begin investigating the riot’s casualty, Bellick interrogates Westmoreland, who maintains silence. In retaliation, Bellick kills Westmoreland’s cat, prompting Westmoreland to finally agree to help Scofield while simultaneously orchestrating Bellick’s blame for the break room incident. This allows Scofield’s crew access to repair the room and continue their tunneling work, reluctantly accepting T-Bag into their ranks.

The conspiracy deepens as Donovan and Savrinn survive an assassination attempt that claims her neighbor’s life. Kellerman and Hale frame LJ for his mother and stepfather’s murders while Vice President Reynolds is revealed as Kellerman’s superior. When T-Bag’s cellmate begs Scofield for help, Scofield’s hands are tied by T-Bag’s knowledge of the escape plan, leading to the cellmate’s suicide. A new inmate, David “Tweener” Apolskis (Lane Garrison), arrives whom Scofield protects from T-Bag’s attention.

Prison politics intensify when Bellick informs Abruzzi about Falzone’s (Al Sapienza) missed payments, suggesting a power play to install Fiorello (Peter J. Reineman) as leader. Abruzzi, frustrated by his inability to locate Fibonacci, retaliates by attacking Gus Fiorello (Peter J. Reineman). Dr. Tancredi’s investigation into Scofield’s background reveals his unique condition of minimal latent inhibition, explaining his exceptional empathy and intelligence.

The plot continues to evolve as Donovan and Savrinn seek refuge at his father’s woodland home, where Terrence Steadman’s widow suggests his business partners may be behind his murder. LJ evades capture and joins forces with Donovan and Savrinn. When C-Note discovers the escape tunnel, he leverages this knowledge to join the team. Scofield and Abruzzi orchestrate an elaborate deception, providing Falzone with false information about Fibonacci’s location, leading to Falzone’s arrest and Abruzzi’s reinstatement as PI chief.

As the escape plan progresses, Scofield and Sara develop a closer relationship. Meanwhile, Vice President Reynolds brings in Quinn (Michael Gaston), a company operative, to oversee operations. Quinn’s investigation leads to Sebastian Balfour’s murder and threatens to expose Donovan’s location. Scofield’s past comes to light when his wife Nika Volek (Holly Valance) delivers a credit card that Bellick discovers through coercion. Using this card and a recovered watch, Scofield creates a timing device to monitor guard rotations around the infirmary.

Westmoreland reveals his true identity as D.B. Cooper and joins the escape plan as he faces his daughter’s imminent death. As the tunnel nears completion, Scofield realizes time constraints will force them to leave one team member behind. Though they plan to exclude T-Bag, he maintains his position by threatening external exposure through his cousin. Meanwhile, Quinn’s pursuit of Donovan, LJ, and Savrinn ends with Quinn trapped in a well where Kellerman and Hale leave him to die.

The situation grows more complex when Abruzzi’s men locate T-Bag’s cousin but accidentally kill both him and his young child during the confrontation. Though guilt-ridden, Abruzzi pledges to protect T-Bag, who feigns redemption before attempting to slash Abruzzi’s throat. Meanwhile, Lincoln is forced to attack CO Geary (Matt DeCaro) to protect the team’s plan, resulting in his placement in solitary confinement.

Personal dramas unfold as Maricruz reveals her pregnancy to Sucre while Bellick coerces Apolskis into spying on Scofield to avoid punishment for stealing from Geary. The conspiracy deepens when Agent Hale attempts to confess to Donovan that Steadman is alive but is murdered by Kellerman before he can reveal more details.

A significant development occurs when Abruzzi is transported out of the prison by helicopter, seemingly resolving the team’s overcrowding issue. With Lincoln in solitary and Pope denying Scofield’s requests to see him, Scofield considers abandoning the escape. However, the team insists on proceeding without Lincoln. Scofield manages to smuggle medication to Lincoln with specific timing instructions while C-Note coordinates with his brother-in-law—the only family member aware of his imprisonment.

The escape attempt faces a major setback when the team discovers their target pipe has been replaced with a reinforced version. They do not retreat but instead devise another plan when a guard approaches. Meanwhile, Donovan and Savrinn arrange for an appeals court hearing while Scofield learns from Westmoreland that there could be delays regarding Lincoln’s execution due to procedural issues rather than an electric chair malfunction.

In court, Donovan’s testimony is undermined when government counsel provides documentation showing no record of agents named Kellerman or Hale in the Secret Service. As Lincoln’s execution date approaches, he makes peace with his fate and bids farewell to his loved ones. However, it is dramatically halted when Lincoln recognizes their father in the observation room. The judge cites discrepancies in coroner’s reports and orders a new examination.

Seeking an alternative escape route, Scofield attempts to navigate through the institution to the infirmary while wearing a guard uniform. Though he succeeds in finding a path, he suffers burns that damage a crucial part of his tattoo. Dr. Tancredi discovers guard uniform fragments in his burned tissue, raising suspicions.

The story explores the characters’ pasts through meaningful revelations. Three years before the present events, Scofield enjoyed professional success while Lincoln faced mounting debts. At first, Scofield held deep resentment toward Lincoln, believing he had squandered their life insurance money. He later discovered that Lincoln had borrowed money to ensure his younger brother’s success, keeping this sacrifice hidden all along. Upon learning this truth, Scofield felt compelled to develop his intricate prison break scheme.

The narrative also uncovers the complex histories of other key players. Before joining Fox River’s medical staff, Sara battled and overcame her own addiction struggles. Sucre landed in prison after a failed robbery attempt, which he undertook to buy an engagement ring for Maricruz—a situation orchestrated by Hector. C-Note’s path to imprisonment began when he was discharged from military service for reporting prisoner abuse, subsequently turning to drug trafficking as a means of survival. The story also unveils T-Bag’s disturbing background, which includes heinous crimes committed against children.

When Scofield refuses to explain his burns, Pope places him in solitary confinement, where he begins to deteriorate mentally. The crew scrambles to conceal their tunnel, with Sucre taking the fall by covering the hole and attempting a diversionary escape through the courtyard. Though Apolskis informs Bellick about the crew’s activities, the evidence is concealed in time.

As punishment for his failed intelligence, Apolskis is assigned to a cell with a less dangerous inmate named Avocado (Daniel Allar). The plot culminates with Scofield feigning illness to contact Haywire about his tattoo, though Haywire has no memory of it. Meanwhile, LJ discovers Kellerman’s identity through Quinn’s final message and attempts to confront him, only to be arrested by police.

As the escape plan enters its final phase, Scofield prevents Haywire from taking medication, allowing him to recall and sketch the missing tattoo section. When CO Geary attempts to auction off Scofield’s cell, the crew tries various methods to raise money but ultimately fails. Sucre recruits his cousin Manche Sanchez’s help by promising him a place in the escape plan.

The prison atmosphere changes when Scofield names Geary as his attacker, leading to the correctional officer’s dismissal. During a transfer, Lincoln’s father Aldo emerges unexpectedly but does not free him; instead, he reveals dark truths—a shadowy organization called the Company controls the U.S. government and used Lincoln as punishment for Aldo’s defiance. Though Lincoln soon returns to solitary confinement, this knowledge expands the conspiracy surrounding his case.

The escape plan encounters multiple challenges. Apolskis endures abuse from his cellmate Avocado and refuses to steal Scofield’s key. Their attempt to duplicate the infirmary key fails after Sara catches the deception and replaces the locks. Upon returning to prison after surviving his injury, Abruzzi reveals he had been controlling Savrinn, who monitored Donovan under his command.

Westmoreland suffers fatal injuries while stopping Bellick after Apolskis confesses the escape plan to him. This forces Scofield to schedule the escape for that night. He implements various safety measures, including using detergents to mask their scent from tracking dogs. During a crucial interaction with Sara, Scofield shares details of the escape plan and begs her to leave the infirmary door unlocked.

During the prison break, Scofield forces Pope to approve Lincoln’s transfer by holding him at knifepoint. Eight prisoners—Scofield, Burrows, Sucre, Abruzzi, T-Bag, Franklin, Apolskis, and Haywire—escape using improvised white uniforms and a cable, with Sanchez falling during the wall crossing. Complications emerge when T-Bag handcuffs himself to Scofield and swallows the key, compelling Abruzzi to amputate T-Bag’s hand in a warehouse to facilitate Scofield’s escape.

The season concludes with multiple dramatic turns: the president dies, Reynolds ascends to the presidency, and Sara lies unconscious from an overdose. Meanwhile, Donovan discovers Steadman alive in Montana, Apolskis escapes in an animal transport van, and Haywire flees by bicycle. T-Bag continues running despite his injury as law enforcement pursues the escapees, bringing the first season of Prison Break to a tense and suspenseful close.
